Inpatient Hospital Services Services must be medically necessary and provided: In a licensed hospital To hospital inpatients Under the direction of a physician For diagnosis or treatment Outpatient Hospital Services Services must be medically necessary and: Must be diagnostic, therapeutic, rehabilitative, and palliative. Furnished to hospital outpatients. Furnished by or under the direction of a physician. Furnished by a licensed hospital facility. Physician Services Services must be medically necessary and provided by medical or osteopathic doctor in the doctor's office, a hospital, a skilled nursing facility, or elsewhere. Prescriptions You can receive up to three prescriptions for drugs per month. New and refilled prescriptions count equally towards this limit. A physician or other licensed practitioner must prescribe the drug within the scope of practice under law. Over the counter medications are not covered by the CIHCP. Laboratory and X-Ray Services These are technical laboratory and radiological services ordered and provided by or under the direction of a physician (M.D. or D.O) in an office or similar facility other than a hospital outpatient department or clinic. An independent laboratory must be certified. Skilled Nursing Facility Services Services must be medically necessary, ordered by a physician, and provided in a licensed, skilled nursing facility that provides daily services on an inpatient basis. *Counts toward maximum 30 inpatient day limit. Rural Health Care Clinic Services Services must be provided in a Medicare (Title XVIII)-certified and approved rural health clinic, and must be provided by a physician, a physician assistant, a nurse practitioner, a nurse midwife, or other specialized nurse practitioner. Family Planning Services These are services that enable individuals to limit family size or space their children and to prevent or reduce births out of wedlock. Women between the ages of 18-44 must access these services via eligibility for the Texas Women's Health Program. Immunizations These are given when appropriate, CIHCP will reimburse for immunizations when they are not available through the local public health department. Medical Screening Services These are screening services, including, but not limited to blood pressure, blood sugar and cholesterol. Annual Physical Examination These are provided once per calendar year by a M.D., a D.O., a P.A., or an Advanced Practical Nurse (APN) if they are within the scope of their practice established by the Board of Nurse Examiners and published in the Texas Administrative Code. Associated testing such as mammograms can be covered with a physician referral. Additional Services The following is a list of the additional services your county has chosen to provide. All medical procedures and services must be medically necessary. Some require an actual letter of medical necessity from your doctor. In addition, some services may require a physician's referral while others may need prior authorization from CIHCP. Contact your caseworker for more information and to see if you meet the requirements prior to receiving any of the below listed services. Failure to do so may result in you being responsible for the medical bill. Free Standing Ambulatory Surgical Center Services Colostomy Medical Supplies and epuipment Counseling Services Dental Care Diabetic Medical Supplies and Equipment Durable Medical Equipment Home and Community Health Services Physical Therapy Vision Care, including Eyeglasses Limited Medical Services CIHCP has a maximum benefit of $30,000 or 30 days inpatient services each physical year from September 1st - August 31st. The following services are not covered by CIHCP unless the following conditions are met. Abortions, unless the attending physician certifies in writing that, in professional judgement, the mother's life is endangered if the fetus is carried to term or that the pregnancy is related to rape or incest. Outpatient Psychiatric Services that exceed thirty visits during a calendar year Private Inpatient Hospital Room, unless it is documented that a critical or contagious illness exists or that no other rooms are available. Treatment of Flat Feet, unless it is documented that it is medically necessary and not for cosmetic reasons. Services Not Covered Under CIHCP Autopsies Chiropractors Ground Ambulance Transport Services Separate fees for completing or filing a claim under CIHCP Services provided outside the United States Services provided by an immediate relative or a household member.
